Title: The use of Mepilex(R) Lite dressings for the management of radiation-induced skin reactions

Description: Breast cancer is the most common malignancy for women in New Zealand. Most of these women will receive radiation therapy, and skin reactions (erythema/dry desquamation/moist desquamation) will occur in 80–90% of these patients by treatment completion. To date, there is no standard treatment for radiation-induced erythema so practice tends to be based on historical and anecdotal evidence.

In 2008, the Dunedin Radiation Department began investigating the clinical efficacy of Mepilex-Lite dressings in reducing radiation-induced erythema in women with breast cancer. These absorbent, self-adhesive dressings consist of a thin, flexible sheet of hydrophilic polyurethane foam bonded to a water vapour-permeable polyurethane film backing layer. The contact surface of the dressing is coated with a soft silicone adhesive layer which does not contain any added chemicals. It adheres to healthy skin, thus ensuring the dressing remains in position but crucially for the radiation field, does not cause trauma to that skin on removal. Furthermore, it provides a moist wound-healing environment. The material does not add or react to chemicals in or on the skin, does not stick to wounds and can be left on the skin for several days. It is currently used in New Zealand for the treatment of burns and slow-healing wounds.

This study is the first clinical trial that has demonstrated that Mepilex Lite dressings decrease the severity of radiation-induced erythema in breast cancer patients treated with radiation therapy. Not only does this increase patient comfort and quality of life, it also reduces the patient's chances of developing long-term consequences of acute skin inflammation such as scarring.

Noelle - Clinical Charge RT and Professional Director, Southern Blood and Cancer Service

Noelle is a radiation therapist (RT) who is originally from the UK. Although she initially trained and worked as a teacher, she retrained to become a radiation therapist at the tender age of 35. Whilst in the UK she worked in several specialist RT roles including that of a Macmillan RT and then later as a specialist Head and Neck RT. In 2002 she immigrated to New Zealand and has spent the last nine years working for Dunedin Oncology (now the Southern Blood and Cancer Service). She started her time there as a treatment supervisor but in 2005 she was appointed to the role of Clinical Charge RT and Professional Director and has been in charge of running the department ever since. In this role she has been able to indulge her passions of patient care, interdisciplinary collaboration and innovative practice/research development – as is evidenced by their shared care model of patient care - thanks to some forward-thinking and equally passionate colleagues.

Jo, Clinical Nurse Specialist in Oncology for the Southern Blood and Cancer Service

Jo has worked in Oncology for the past 20 years in a variety of settings. She has been an Oncology Nurse in rural practice, a chemotherapy treatment nurse and worked in Radiation Therapy treatment units both in New Zealand and overseas. At present she is the Clinical Nurse Specialist in Oncology for the Southern Blood and Cancer Service. Her current role gives her both a clinical focus and also the opportunity to develop nursing in the service, to focus on improving patient care and to work in a very collaborative model of care with other disciplines. She has also been privileged to help to develop guidelines for the service and to help undertake research that is designed for improved patient care at the very coalface of treatment.

Title: Advanced Dressing Technology in the Management of Wounds and the Risk of Infection

Description: Modern wound dressing technologies are widely understood to offer clinical advantages over traditional materials such as gauze. Conditions required for effective wound management that are offered by advanced technologies include a moist wound environment, manage wound exudate, contribution to pain management1, and antimicrobial effect2. Silver has proven, over years of investigation, to be a broad spectrum antimicrobial agent with a good safety profile and minimal probability that microbial resistance to it will develop2. Its use in medical and nonmedical fields is increasing. The antibacterial action of silver is expressed through interaction with multiple biological sites in bacteria3, and recently‐published data suggest that it also adversely affects the structure of biofilm4. The antibacterial effect of silver is expressed differently in different products5. When used in wound management in a clinically appropriate manner based on assessment, diagnosis and due consideration of clinical need matched with product indications, dressings containing silver provide a valuable weapon in the battle against wound infection6. This session will address the scientific and mechanistic basis for the use of silver‐containing advanced wound dressings in the management of the risk of wound infection.

Speaker Bio:
Jerry has an excellent grasp of research relating to wound dressings and to the fundamentals of Wound Healing Physiology. His interests are pathophysiology of chronic wounds, with a particular focus on early stage innovation.  He has  an extensive Lab-based background on antibiotics and antifungals and their interaction with host defense mechanisms.  Dr Jerry Hutchinson was awarded a BSc (hons) in Microbiology in 1977 and a Ph.D. in 1981 (University of Leeds). From 1980 to 1983 Dr Hutchinson conducted post-doctoral research at the Department of Microbiology, Birmingham University. In 1983 he joined the Microbiology Department of ER Squibb’s International Development Laboratory. In 1985 he transferred to the newly-founded ConvaTec Biological Research laboratory focusing on wound healing, which developed over time to become the ConvaTec Global Development Centre (GDC). He has served at GDC as Director in appointments related to the biology and pathophysiology of wounds, and is currently Director, Human Physiology and Innovation, with responsibility for pathophysiology, and for ConvaTec’s technical innovation process across the business. Jerry is based in ConvaTec’s GDC facility in Deeside, North Wales. and other conditions related to ConvaTec business.  Current memberships include the Wound Healing Society, Society for General Microbiology, British Burn Association.

John M Macdonald, MD, FACS, is a native of Pittsburgh, Pennsylvania.   He received a Bachelor of Science degree at the University of  Notre Dame and his Medical Degree at the University of Pennsylvania, School of Medicine.  Internship served at Jackson Memorial Hospital, University of Miami.  Military duty as Captain in the United States Air Force.  Residency in general and thoracic surgery at the University of  Pittsburgh Medical Center.  Fellowship in thoracic and cardiovascular surgery as Senior Registrar, St Bartholomew`s Hospital, London England.
 
Dr Macdonald is Board Certified in general surgery and thoracic surgery.  From 1971 until 1999, he practiced thoracic and vascular surgery in            Ft Lauderdale, Florida.  Over the past  20 years, Dr Macdonald has been extensively involved in the clinical and research aspects of wound care and lymphedema.  He has lectured extensively both in North America and internationally.  Dr Macdonald has authored numerous articles and text chapters relating to both lymphedema and wound related lymph stasis.  He is the editor of the recently published, WHO White Paper--"Wound and Lymphoedema Management"  In 2002 he was appointed to the faculty of the Department of Dermatology and Cutaneous Surgery at the University of Miami, Miller School of Medicine.

Dr Macdonald is currently the medical director for the University of Miami Lymphedema Clinic at Jackson Memorial Hospital.   He is the Past President of the Association for the Advancement of Wound Care (AAWC).   Dr Macdonald is the founder and Secretariat for the--World Alliance for Wound and Lymphedema Care, (WAWLC) based in Geneva, Switzerland. 

In the first two days post earthquake, January 12, 2010,  Dr Macdonald established the wound care program in the University of Miami , Project Medishare,  Haitian Relief Tent Hospital in Port-au-Prince.   He is the Medical Director of this program now located at the at the  Hospital Bernard Mevs in Port-au-Prince, Haiti.

Title: Using the Intermittent Pneumatic Compression System - FLOWTRON HYDROVEN for the Treatment of Intractable Venous Leg Ulcers.

Description: The mainstay of treatment for venous leg ulceration as we all know is compression – usually compression bandages and then stockings once the ulcers have healed. Some patients are unsuitable for compression bandages/stockings due to 'body habitus' i.e. very large legs or deformed legs or both. Some patients state that compression bandages are too painful to tolerate and so the leg ulcers deteriorate and the patient has frequent admissions to hospital with cellulitis.

Mechanical compression using the Intermittent Pneumatic Compression System - FLOWTRON HYDROVEN has been found to be well tolerated by patients and is a cost effective alternative to compression bandages and stockings. This workshop will look at both the theory and the practical application of mechanical compression.

Carol has worked as a Stomal Therapist for the last 22 years. Her role has encompassed wound management for 12 years.
Carol has a Certificate in Stomal Therapy Nursing, Degree in Nursing and a Masters in Adult Education.
She is an active member of the Australian Association of Stomal Therapy Nurses (AASTN) holding several roles over the last few years including Journal Editor and State Representative/chairperson. Carol is a member of  The World Council of Enterostomal therapists and is on the
education committee of this organization. Carol is one of the writers of the Graduate Certificate in Stomal Therapy Nursing Distance Education program conducted through the College of Nursing and is employed as a tutor by the College. Carol has been the preceptor to many student stomal therapy nurses over the years in the fields of both stoma care and wound management.
Carol has presented papers at several International and National Conferences and is the author of many journal articles.
